SUMMARY: Coordinate and provide primary care services to patients with HIV disease, in collaboration with the center’s medical staff. Maintain individualized documentation/care plan for HIV patients based on ongoing assessment and identify services needed. Provide ongoing patient education to empower patients to take an active role in managing their health.
