SUMMARY
The
Case Manager provides assessments and discharge planning in coordination with
the providers to patients and families to assist them and the health care team
in coping with patient's illness, diagnosis, treatment, and/or life situation,
including emotional, mental, and substance abuse disorders, and other financial
or social needs.
